Comments: Received a complaint of smoking. Made field visit on 2-11-2016 and spoke with Dolly. When I entered the kitchen I observed Dolly had a unlit (partially smoked) cigarette in her mouth preparing food in the kitchen (upon seeing me she immediately removed the cigarette from her mouth). I spoke with Dolly about the smoking complaint and about not smoking in the food service establishment. During this conversation I also observed a food worker using his bare hands packaging rolls into small plastic baggies. Rolls were discarded and he put gloves on.
