Comments: All violations must be corrected and are subject to enforcement action if not addressed in a timely manner. Facility was storing multiple temperature controlled pizzas at room temperature on display counter. Pizzas including broccoli, Buffalo chicken and sausage and peppers were temped using a metal stem probe thermometer at 72°F. Facility did not have Time as a Public Health Control Waiver (TPHC) but was using a tracking system to discard pizzas after 2 hours. Inspector provided TPHC waiver application. Please submit application to NYS DOH Glens Falls District Office within 2 weeks.

Comments: Re-inspection conducted to verify thorough cleaning done of facility since last inspection. Hoods and walls decreased. Floors had food debris but not beyond a reasonable amount for how long facility has been open today. All coolers and reach-in freezer soiled with spilled foods. Operator cleaned to inspectors' satisfaction at time of re-inspection. Professional cleaning company due to return for another deep clean in December 2016. Chlorine residual = 1.31 when checked at three-bay sink in rear kitchen. Facility logs show daily monitoring of chlorine residual.
